Expert care in people’s homes

The Marie Curie Hospice at Home service provides expert, round-the-clock support, enabling more people with a terminal illness to die in the place of their choice. In 2016-17, 5,046 visits were made in patients’ homes, an increase of 746% from 2014-15, before the service began.

This partnership with Marie Curie is a huge step forward in our mission to provide excellent end of life care to patients. The service provides care at home to some of our most vulnerable patients through an additional, specially trained and dedicated team. It means patients who wish to remain at home are supported and receive appropriate care suitable for their individual needs.

End of life care for patients with Fast Track funding

Marie Curie Seniors Nurses ensure patients in Dorset who are eligible for Continuing Healthcare (CHC) Fast Track funding get the right care, in the right place, at the right time. In 2016-17, 1,600 referrals were received for Fast Track funding at home and in nursing homes which the Marie Curie team co-ordinated, and 57,000 hours of care were delivered for those patients.

The Marie Curie Fast Track service in Dorset provides effective and efficient co-ordination and ongoing review to ensure patients are getting the right care packages to meet their needs. Marie Curie brings its considerable expertise to the partnership and provides assurance that the care people receive at the end of their lives is of the highest quality. It also offers flexibility, adapting the service to the constantly changing healthcare environment we work in.

Award-winning support for patients and families

The award-winning Marie Curie Companions volunteer service provides one-to-one emotional support to patients who are dying, and their families, 365 days a year. More than 350 patients have been supported by 30 volunteers since July 2014.

The Marie Curie Companions service is about a community walking a path with people in the last days and hours of their lives, and their loved ones. By offering time, companionship and much-needed emotional support to patients and families, these volunteers also reduce stress among our staff. Their presence on our wards allows nursing staff to focus on all clinical priorities, knowing that that includes the holistic needs of this special group of patients.

Tailored, integrated care services

Our innovative, jointly-funded Marie Curie @ Northumbria service partnership ensures people with palliative care needs have co-ordinated, responsive and patient-centred specialised care and support in hospitals and the community. Patients were seen by our hospital teams 2,620 times in 2016, an increase of 216% since before the partnership in 2014.

Northumbria has invested significantly and creatively in palliative care to create the largest integrated palliative care service in the country. The focus of this partnership is to increase our capacity and quality of service by delivering more responsive services, as well as new roles that work alongside our existing teams in the community, hospitals and our palliative care units.
