Community bereavement support referral form for professionals
Marie Curie Newcastle Community Bereavement Support Service is designed to meet the needs of bereaved people in the community who have lost someone due to coronavirus, or whose grief process has been impacted by coronavirus.
Our aim is to offer emotional support and counselling, via one-to-one telephone and one-to-one virtual appointments via Microsoft Teams.
The below form is for medical and service professionals to refer patients or clients (if you want to refer yourself to the service, please use this form instead).
Please note that due to current restrictions our bereavement support service is provided by telephone or virtual consultations through Microsoft Teams. Both individual and group support is available.
Referral criteria
Someone is suitable for the service if they are:

an adult (over the age of 18) who has been affected by a death caused by or impacted by coronavirus

able to access telephone or online support

able to engage with the therapeutic process.
Unfortunately we are not able to work with:

adults with substance misuse or alcohol dependency issues

those presenting as a risk either to themselves or others

those who are already receiving psychological therapy from another service.
What we offer

Monthly emotional support sessions delivered by trained, supervised bereavement volunteers

Depending on complexity/level of need, some counselling is also available

AAG (Adult Attitude to Grief scale, Machin) used to assess, inform and evaluate intervention

Please note that in current circumstances all support is delivered via telephone or Teams online.
